To enroll in the program, just follow these simple steps:
- Complete the attached Enrollment Form. (downloadable pdf)
- Mail or Fax a copy of your Professional License and Enrollment Form to 949-623-0990. Once this is received, we will send out your professional information packet, as well as a pricing sheet.
- Begin ordering!
As a Partner in our HCP Program, you’re entitled to great prices without having to place large orders. Your minimum order is a single bottle of a product! You can get any quantity you’d like, mix and match the products to suite your needs as well as those needs of your patients. Orders are quickly and efficiently processed to ensure product delivery within 5-7 business days.
